- Doctoral (Non-PhD) Committee/Revise Doctoral (Non-PhD) Committee
- Use one of these forms either to designate faculty members to serve on a committee for a EdD, DNP, DrPH, or DSW program or to revise an established committee.
- Report of Final Examination/Defense of Dissertation/Project/Capstone
- This form is to be completed by the doctoral committee when the graduate student has completed the final examination for the degree or has defended the dissertation, project or capstone.
- Request for Concurrent Master’s Degree
- This form is only to be used by PhD students who intend to remain active in the PhD degree program and complete the master’s degree along the way to the PhD.
- 600 Continuous Enrollment Exemption
