Master’s or Specialist Degree

Admission to Candidacy—Master’s or Specialist in Education
The Admission to Candidacy application establishes the coursework required to graduate with a degree in a chosen program.
Revised Admission to Candidacy—Master’s or Specialist in Education
Use this form when you have submitted your Admission to Candidacy form for a master’s or specialist in education degree, but need to revise your candidacy.
Report of Final Examination/Defense of Thesis/Project/Capstone
This form is to be completed by a master’s, educational specialist, or DSW committee when the candidate has completed the final examination or defended the thesis, project, or capstone.

Doctoral Degree

Admission to Candidacy—Doctoral
The Admission to Candidacy application establishes the coursework required to graduate with a degree in a chosen program.
Revised Admission to Candidacy—Doctoral
Use this form when you have submitted your admission to candidacy for a doctoral degree, but need to revise your candidacy.
PhD Committee/Revise PhD Committee
Use one of these forms either to designate faculty members to serve on a PhD committee or to revise an established PhD committee.
Doctoral (Non-PhD) Committee/Revise Doctoral (Non-PhD) Committee
Use one of these forms either to designate faculty members to serve on a committee for a EdD, DNP, DrPH, or DSW program or to revise an established committee.
Schedule of Dissertation/Capstone Defense
This form is used by doctoral candidates who are preparing to schedule their dissertation/capstone defense, as part of the requirements for graduation. (Preview the Form)
Report of Final Examination/Defense of Dissertation/Project/Capstone
This form is to be completed by the doctoral committee when the graduate student has completed the final examination for the degree or has defended the dissertation, project or capstone.
Request for Concurrent Master’s Degree
This form is only to be used by PhD students who intend to remain active in the PhD degree program and complete the master’s degree along the way to the PhD.
600 Continuous Enrollment Exemption
Doctoral students who have started taking course 600 and wish to do an internship/practicum can petition to be exempted from the Continuous Enrollment requirement. (Preview the Form)

Thesis/Dissertation

Thesis/Dissertation Approval
This form is used by Doctoral candidates and Master’s candidates in a thesis program to receive approval of their thesis or dissertation, as a requirement for graduation.
Initial Embargo Request
Students with significant concerns related to sensitive or classified information, patents, and potential publishers’ restrictions may request an embargo after the conferral of their degree to delay public release of the thesis or dissertation. (Preview the Form)
Embargo Renewal Request
This form is to be used by a former student wishing to extend the embargo of the thesis or dissertation. (Preview the Form)
Request for Use of a Foreign Language in Preparation of the Thesis or Dissertation
This form is to be used to request permission to write a thesis or dissertation in a language other than English. (Preview the Form)

Second Deadline

Second Deadline Graduation Application
This form is to be used by graduate student candidates who were unable to meet the deadline dates for the current semester to request to graduate in the following semester. (Preview the Form)

Registration

Late Change of Registration
Students should use this form when needing to request a change to their registration, specifically adding and/or dropping a course, if the deadline for dropping and adding courses for the desired term has passed.

Graduate Certificate

Admission to a Graduate Certificate Program
This provides access to apply for graduate certificate programs, which provide focused areas of study.
Graduate Certificate Course Verification
Use this form if you earning a Graduate Certificate to verify that you have completed the coursework necessary to receive the certificate. (Preview the Form)

Graduate Restart Program
The Graduate Restart Program is intended to assist former UT graduate students whose cumulative UT graduate grade point average (GPA) is below a 2.00 to re-enroll in a degree program or graduate certificate program without having to overcome the burden of previous unsatisfactory academic performance. Permission to Repeat a Graduate Course
This form is for graduate students wishing to repeat a course in which they earned a grade of D or F. (Preview the Form)
Graduate Student Leave of Absence
This provides a request for a Leave of Absence (LOA), a temporary cessation of study which may be granted for up to two (2) years for medical reasons and other emergencies. (Preview the Form)
Reinstatement Request

Request for Letter of Degree Completion

Senior Requesting Graduate Credit
This application is for undergraduate seniors who intend to apply to graduate school and are registering a course for which graduate credit is offered. (Preview the Form)
Permission to Take a Graduate Course Overload
This form is for graduate students wishing to take course hours beyond the 15 credit hours standard for a single term. (Preview the Form)
