This form is to be completed by a master’s, educational specialist, or DSW committee when the candidate has completed the final examination or defended the thesis, project, or capstone.

This form is only to be used by PhD students who intend to remain active in the PhD degree program and complete the master’s degree along the way to the PhD.

This form is used by Doctoral candidates and Master’s candidates in a thesis program to receive approval of their thesis or dissertation, as a requirement for graduation.

The Graduate Restart Program is intended to assist former UT graduate students whose cumulative UT graduate grade point average (GPA) is below a 2.00 to re-enroll in a degree program or graduate certificate program without having to overcome the burden of previous unsatisfactory academic performance.
