Student Health Services

Registered Students are eligible for care at the Student Health Services, located at 1818 Andy Holt Avenue. No health insurance is required and students pay minimal or no charges, depending on the services rendered.
UT pays for health insurance for qualified TAs/RAs. Spouses and dependents may be covered by Student Health Insurance, but are not eligible for treatment at the Student Health Services.
Health Forms
Complete and submit the Immunization Requirements/Record and Medical History Questionnarie forms prior to classes. The forms can be found at http://web.utk.edu/~shs/Forms.html. Send the completed forms to:
